Gage Calibration Intervals
Per ISO/IEC 17025, Clause 7.8.4.3, neither Accu-Chek, Inc.’s calibration certificate nor calibration label shall recommend calibration intervals for your equipment. Calibration due dates do not imply continued conformance to specifications. It is your responsibility to inform us what calibration interval you want applied to each UUT. Please choose a 1 month, 3 month, 6 month, or 12 month calibration interval. Unless otherwise notified in writing on your Purchase Order or by other means, Accu-Chek will assign a calibration interval based on industry-accepted practices.
Standard Services
Accu-Chek calibrates all inspection, measuring, and test equipment (IMTE) to either manufacturer’s specifications, Federal/Military/ANSI specifications or, if requested, we can perform calibrations to customer specifications. If you have a preference, it must be noted on your purchase order. We calibrate all IMTE using written procedures and standardized data sheets. All of our work is performed by trained technicians and is continually monitored by both internal and external ISO 17025 auditors.
Your test instruments will be calibrated using standards with accuracies traceable to the (SI) via calibration laboratories accredited by signatories to the International Laboratory Accreditation Cooperation (ILAC) Mutual Recognition Arrangement (MRA) and by National Metrology Institutes (NMIs) – such as the National Institutes of Standards and Technology (NIST) – under the International Committee of Weights and Measures (CIPM) MRA or derived from natural constants, derived from ratio comparisons, or compared to consensus standards. All gages sent in-house for calibration are allowed sufficient time to soak in our temperature controlled labs until thermal stabilization has occurred.
Our quality system complies with ISO/IEC 17025, ANSI/NCSL Z540-1-1994, ISO9001, IATF 16949, ISO10012-1 and MIL-STD-45662A. The certificates for your equipment will comply with ISO 17025, IATF 16949 and ANSI/NCSL Z540-1.
Units found out of tolerance during calibration are documented with an Out of Tolerance (OOT) Notice. This document provides you immediate notification that an out of tolerance condition exists and explains the defect and the corrective action(s) taken. If the unit could not be calibrated within acceptable accuracy limits a “Do Not Use” sticker may be affixed to the unit to prevent inadvertent use.
Standard and Accredited Certificates
A standard calibration is completed with a statement of compliance to be issued without considering uncertainty in determining compliance to specification which shall contain the results and the MU. In these instances, the customer assumes partial risk of determining compliance (rather than the entire risk being assumed by the calibration laboratory). The certificate shall and does include the following statement:
“The statement of compliance in this certificate was issued without taking the uncertainty of measurement into consideration. The customer shall assess the results and uncertainty when determining if the results meet their needs.”
The statement of conformance to specification for the UUT is based on Accu-Chek, Inc.’s best estimate of the measurand being equal to or less than the specified accuracy of the UUT. If the measured value falls outside the stated specification, the UUT will be assumed to be out of tolerance.
Per ISO/IEC 17025, Clause 7.8.6.2, for each unit under test (UUT) Accu-Chek, Inc. provides a Certificate of Calibration that states the condition of the instrument relative to the manufacturer’s tolerance (or that designated by the customer). The “As Found Condition” and “As Left Condition” statement will be one of the following: “As Reported”, “Fail”, “Fail, Adjusted”, “Fail, Repaired”, “Functional Failure”, “Pass”, “Pass, Adjusted”, “Pass, Repaired” and “Undeterminable”.
Pricing
Any estimated prices do not cover the cost of any safety training fees that might be required to enter the customer’s facility. If special safety training is required, the customer shall make all arrangements and shall pay for any training fees required. Service Technician training time will be charged at $85.00/hr. Accu-Chek, Inc. should be notified at the time of receiving the Purchase Order of any required training.
Any estimated prices are based on the equipment inventory you have supplied. Accu-Chek reserves the right to change any price listed if the equipment is different than originally stated on the inventory list provided by your facility.

In addition to the calibration costs, travel expenses will be incurred. Travel time and mileage expenses are $1.30/mile for the first technician plus $0.85/mile each additional technician. $125.00 per diem for each technician is also charged when an overnight stay is required. However, travel expenses are normally divided among all customers receiving service unless our technicians must make an emergency trip or specific run to only your facility. ($50.00 minimum)
A $45.00 minimum charge will be assessed to all work performed with the exception being surface plates at $125.00 minimum. Your actual invoice may be different than that estimated above due to changes made to the equipment list or for parts and labor required for any needed repairs. All prices are guaranteed for 45 days after the date of this estimate. Prices are subject to change without notification beyond the 45 day period.
These prices are good for all work scheduled during normal operating hours of Monday-Friday, 7:00am-5:00pm. Any work that must be scheduled during 2nd shift, 3rd shift, Saturday or Sunday is subject to price increases commensurate to the overtime required for our calibration technicians.
Payment Terms and Conditions
Your company will be billed upon the completion of each service and will be due within 30 days of the invoice date. For your convenience MasterCard or VISA are acceptable as well as standard purchase orders. There will be a “convenience charge” for the use of any credit cards commensurate to the processing fees incurred. Late payments will accrue at 21% annualized interest rate beyond net 30 of the invoice date and will include attorney fees associated with collecting unpaid debt.
Cancellation of work in progress will result in a 20% charge against work not completed. Because our pricing structure is primarily unit based, there will be a $65.00/hr charge for any non-productive work (i.e. equipment unavailable, wait time due to customer contact, etc.). A 35% restocking fee will be charged for procured materials in the event of cancellation once a purchase order has been received by Accu-Chek, Inc. All prices are FOB shipping point with freight charges added to the invoice. Responsibility for safe delivery of equipment is assumed by carriers upon acceptance of shipments. All claims for damage must be filed with the carrier.
Accu-Chek, Inc. shall not be liable for loss in any respect regardless of whether customer’s claim is based on contract, tort, strict liability or otherwise. Liability shall be limited to an amount equal to the contract price or that sum of money actually paid by the customer toward the contract price and this liability shall be exclusive. In no event shall Accu-Chek be liable for any loss of customer’s business, revenues, profits, the cost to the customer of any other special, incidental, consequential or punitive damage of any nature, or for any claim against the customer by a third party.
